A Track Cirecuit Operating Device was not fitted to the adjacent line, by Driver E222, after the derailment; as a result the RAIU make the following safety recommendation:
Iarnród Éireann (IÉ) (Railway Undertaking) should brief the relevant staff on the requirements of the IÉ Rule Book (Section M 3.1.2) which states that where emergency detonator protection is not needed, drivers must place a Track Circuit Operating Device on the line(s) concerned to supplement the signal protection.
