As the Points Operator did not appear to have a clear understanding of how to perform his duties
in terms of the hand operation of power operated points as a result of deficiencies in training,
continuous assessment, performance records. As a result the RAIU make the following safety recommendation:
Iarnród Éireann (Infrastructure Manager) should conduct a full review of IMO-SMS-031, ‘Competence Management – Persons required to conduct IM operating duties’ and associated documentation, to identify deficiencies in training, continuous assessment and the recording of performance of duties to ensure that persons carrying out these duties are competent to do so.
