The Points Operator and the Controlling Signalman did not adhere to the strict requirements for
safety critical communications, resulting in misinterpretation of directions by the Points Operator
which lead to further errors. As a result the RAIU make the following safety recommendation:
Iarnród Éireann (Infrastructure Manager & Railway Undertaking) should evaluate the current training, assessment and monitoring of Safety Critical Communications to ensure that communications are carried out to the requirements set out in IÉ Rule Book, and safety critical communications standards IMO-SMS-033 and OPS-SMS-8.1.
