A review of the internal IÉ investigation methods by the RAIU found that the current system of reporting (OOR) has not yet been formalised; there is a lack of consistency in relation to the terms used in the reports and that the reports appear to take a very long time to complete, as a result the RAIU make the following safety recommendation:
Iarnród Éireann (Infrastructure Manager & Railway Undertaking should review the current system of reporting Signal Passed At Danger (SPAD) events so that reports are consistent and published within a set period of time.
