As the previous near miss reporting system, introduced into Iarnród Éireann was unsuccessful, and given that near miss Signals Passed At Danger (SPADs) are occurring, and consequences of SPADs can be serious, the RAIU make the following safety recommendation:
Iarnród Éireann (Infrastructure Manager) should identify safety critical signals and, where the technology exists, introduce a mechanism to monitor the approach speed to these signals; to ensure that near misses are identified and managed.
