In some cases, there is no recording of communications between safety critical staff (e.g. between drivers and signalmen, such as the Signal Passed At Danger at Start & Obtain Instructions Board, Limerick (Limerick), 24th September 2014). As a result, the RAIU make the following safety recommendation:
Iarnród Éireann (Infrastructure Manager) should identify all locations where safety critical communications are not recorded and develop a programme of works for the introduction of recording safety critical communications at these locations.
