The RAIU found that IÉ-IM were slow to respond to RFIs submitted to the RAIU, and in some cases the wrong information was submitted, which extended the RAIU investigation over the twelve-month recommended publication date, as a result the RAIU make the following safety recommendation:
IÉ-IM should introduce processes to ensure that information submitted to the RAIU is correct and submitted within the requested timeframes.
