Basis for Recommendation
Scheduled maintenance (A Exams and a B6 Exam) did not and would not detect latent faults on
autocouplers; given there is a concern in relation to all fleets, the RAIU make the following safety recommendation:

Recommendation
Iarnród Éireann (Railway Undertaking) Chief Mechanical Engineer's Department should review their scheduled maintenance examinations, for multiple-unit fleets, with a view to developing a means to check the connection is correct on the electrical head.
