The Difflin Light Railway (DLR) Safety Management System (SMS) failed to fully identify all the risks associated with operation of their trains; in addition, there were several other issues identified with the DLR SMS; as a result the RAIU make the following safety recommendation:
Difflin Light Railway should review their Safety Management System (SMS), in its totality, and ensure that there are internal monitoring procedures that mandates the periodic checking of application of SMS processes and practises.
