The Difflin Light Railway (DLR) Safety Management System (SMS) failed to fully identify all the risks associated with operation of their trains; in addition, there were several other issues identified with the DLR SMS; as a result the RAIU make the following safety recommendation:
Difflin Light Railway should review their risk assessment process to ensure that all reasonably foreseeable risks associated with the operation of trains are identified and suitable control measures identified.
